Maine Advance Health Care Directive
In Maine, the document that names someone to make medical decisions for you when you cannot speak for yourself is called a advance health care directive. Elsewhere it goes by medical power of attorney or advance directive agent designation: same role, local name. In Maine, a advance health care directive generally must be signed by two witnesses; notarization is not required (Me. Stat. tit. 18-C, 5-803).
