South Carolina Health Care Power of Attorney
In South Carolina, the document that names someone to make medical decisions for you when you cannot speak for yourself is called a health care power of attorney. Elsewhere it goes by medical power of attorney or advance directive agent designation: same role, local name. In South Carolina, a health care power of attorney generally must be signed by two witnesses; notarization is not required (S.C. Code Ann. 62-5-504).
